职位概况
负责为加入计划的病患提供支持照护，鼓励遵从疾病管理并传递健康照护相关信息。安排病患加入计划的时机，确保提供符合服务质量标准的优良病人照护与客户体验。
主要职责
- 执行跨科别病患计划，必要时配合跨区域支援安排卫教或健保事务。
- 根据班表出勤驻诊或依据患者约诊时间提供实地与电话访视服务。
- 遵守法规、公司行为准则及专案SOP，及时通过邮件与电话提供即时支持。
- 为病患提供个性化照护，包括健康指导、激励支持及目标设定。
- 收集并确保病患信息准确，维护个人资料保密，处理药物不良事件通报。
任职要求
- 3至5年相关工作经验。
- 护理或医药相关科系大学学历。
- 具备专案规划、执行与评估经验。
- 有与外部客户互动的经验。
必备技能
- 持有护理师执照。
- 具备护理学校文凭。
